Policies

  • Any missed appointment (No Show) or any cancellation of appointments done within 24 hours of your scheduled session (late cancellation) prevents access to other clients that would want that available time.

    If you need to cancel or reschedule your appointment, please call the office or message through Spruce.

    For established clients, a fee of $75.00 will be charged to the credit card on file for a late cancellation or a No Show.

    For new clients, a fee of $100.00 will be charged to the credit card on file for a late cancellation or a No Show upon request for rescheduling.

    For appointments made within 24 hours (URGENT): This is considered an unusual circumstance. As such, no cancellations will be accepted because you ARE already within the 24 hour period. If you are requesting one, and you cancel any time after making it, the Cancellation Fee WILL apply.

  • We DO NOT provide Emotional  Support Animal letters. There are several community resources that can be easily identified with a simple web search.

  • As we are a small private practice with limited resources, we DO NOT complete long-term disability paperwork

  • Prescription refills require 24-48 hours notice. A $25.00 fee may be required, at the discretion of the provider, for refills outside of appointments. Use Spruce for your refill request. Refills will not be filled if you have not been seen by the provider within three (3) months, missed a scheduled appointment, or do not have a scheduled follow-up appointment. Only enough medication (bridge prescription) will be provided until your next scheduled appointment.

  • It is required for you to keep a valid credit card on file. If you are using one of the following insurances: United Healthcare, Oxford, Aetna, Cigna, Oscar, Blue Cross Blue Shield of Florida, Blue Cross Blue Shield of New Jersey, or Blue Cross Blue Shield of Massachusetts you will receive an email from our billing company, Headway. They will ask you to fill out two forms. They will also require that you keep a credit card on file for your co-pay.

  • Breiter Mental Health & Wellness DOES NOT offer on-call services for emergencies. If such an unfortunate emergency were to occur, please call 911. The Suicide Prevention Lifeline, 988, is now available across the United States as well.

  • Phone calls and messages will be returned within 48 business hours. Calls and messages received after hours or on the weekend will be returned during working hours.

  • An electronic record is kept of services you receive at Breiter Mental Health & Wellness. You have a right to receive a copy of it upon request. You may authorize in writing that copies of the record be released to medical providers you designate at no cost, or may be picked up in person at your expense for a fee. Under certain circumstances where seeing the record may put a client or other person at risk, we may redact certain information in the record and/or require that you review the record in consultation with another healthcare provider.

  • We use Doxy.me for Telehealth appointments. You can find the link below to log into your provider's room. This link does not change or expire.

    Please log in a few minutes before your appointment. The provider can see that you are waiting. Think of it as a waiting room. We understand that at times you may be delayed in getting signed onto your appointment on time. Late starts will result in a shorter session. It is the provider's discretion whether there is enough time remaining to conduct the visit and provide the best quality care. If the provider determines there is not enough time remaining, the appointment will be considered a No Show Appointment. Please be in a private and quiet location during the Telehealth appointments. If you are taking the Telehealth appointment in the car, you should not be driving or moving due to safety and poor reception concerns.

  • The office is located at 506 N. Armenia Ave., Tampa, FL 33609. Please enter through the back door and have a seat. To maintain a calm and peaceful setting, please bring only those who are essential to help with the initial evaluation or follow-up appointment. We understand that at times you may be delayed in getting to your appointment on time. Late starts will result in a shorter session.

  • The below conditions will be grounds for dismissal from the practice with a referral to another practitioner:

    • Within one calendar year – THREE (3) - less than 24-hour cancellations

    • Within one calendar year – THREE (3) no shows

    • Unruly or disruptive behavior, obscene, abusive language, or noncompliance with treatment recommendations as determined by your provider
